Instanet Forms Toolbar Error

If you have loaded a form and the Instanet Forms toolbar on the left appears with the regular adobe acrobat toolbar icons duplicated (see below for an example), click on the ‘Refresh’ button located along the top of your browser or select ‘Refresh’ from the view menu.
